Three steps for your office
- Step 1
Complete the referral form
Download the referral form and either print it or complete digitally and fill in the patient's details, reasons for the referral and any clinical caries noted on your exam. Any X-rays or notes you can share will help us get the anesthesia approval for your patient.
- Step 2
Give it to the patient's family
Hand the completed form to the parent or guardian so they have everything they need when they reach out to us.
- Step 3
The family contacts us. We take it from there.
Our team finds the right surgical location for the child, handles the insurance approval, and schedules treatment.

Who to refer
littleGRINS treats children ages 2 to 8, and children up to age 12 with a clinical diagnosis of a mental or developmental disability, whose dental care can't be completed safely in the dental chair. Common reasons include treatment that was attempted and couldn't be finished, nitrous oxide that didn't help, extensive decay, and medical, developmental or sensory needs.
