Space maintainers

A small appliance that holds space open when a baby tooth is lost too early, so the adult tooth has room to come in.

What it looks like

A small wire loop that saves room for the adult tooth

A band-and-loop space maintainer fits around a neighboring tooth and reaches across the gap, so nearby teeth can't drift in before the permanent tooth arrives.

How it's done

Step by step

  1. 01

    Early tooth loss

A baby tooth is lost early from decay or injury.

  2. 02

    Measure and fit

A band is fitted to a neighboring tooth with a loop across the gap.

  3. 03

    Cement in place

The appliance is cemented so it stays secure.

  4. 04

    Remove later

Once the adult tooth starts coming in, the dentist removes it.

01

Why space matters

If a baby tooth is lost early because of decay or injury, nearby teeth can drift into the gap. The adult tooth may then come in crooked or get blocked. A space maintainer holds the spot open until the permanent tooth arrives.

02

Types of space maintainers

Band and loop

a metal band on a neighboring tooth with a loop that spans the gap

Lower lingual holding arch

a wire behind the lower front teeth that holds space on both sides

Distal shoe

guides a first permanent molar that has not come in yet

03

Caring for a space maintainer

Avoid sticky candy and gum, don't push on the wire with fingers or tongue, and brush around the bands carefully. Your dentist will check it at each visit and remove it once the adult tooth is coming in.

Does every lost baby tooth need a space maintainer?

No. It depends on which tooth was lost, your child's age, and how soon the adult tooth is expected. Your dentist will evaluate this on X-rays.

How long does a space maintainer stay in?

Until the permanent tooth begins to come in, which can be several months to a few years.
